As a member dentist of the Premier Dental Group you have agreed to treat, at a reduced fee, patients covered by various insurers, third-party administrators and discount fee plans. This makes available to you, the dentist, any persons insured or covered by those insurers by a plan that designates Premier as the preferred provider network.

The following tables list insurers, third-party administrators and discount fee plans who have contracted with the Premier Dental Group for their dental network. NOTE: Most member ID cards will have the necessary phone numbers also. ALWAYS CHECK THE PATIENT’S ID CARD.

Network Descriptions

PREMIER CLASSIC NETWORK (PDG)
The Premier Classic Network is our most comprehensive network, with over 15,000 employer groups and over 350,000 patients as members to service in the upper Midwest. We also currently contract with 20 insurance companies.

DHA-PREMIER NETWORK
DHA-Premier is a larger, national network comprised of dentists who are members of The Premier Dental Group and/or of DHA. Employers who select the DHA-Premier Network may do so through either DHA or Premier. Dentists may be members of either DHA, Premier, or both.

PREMIER CLASSIC PLUS NETWORK
The Premier Classic Plus network is proprietary network currently used by UnitedHealth Dental/Group.
